Yes the way see it. When there is compelling reasons (which stand out even with countdown), not just because he is being too slow. But why would you not use the countdown?
Example: keeper places the ball on the spot for the kick but then walks to it, picks it up and punts it to the next suburb.
OR, picks it up and walks to the other end of 6 yard box for no apparent reason, for the second time.

I wouldn’t caution the second one. It’s just a corner kick for taking too long. First one I can see—though it is more about disrespecting than the delay even if we’re written up as DR (Though I might just start counting on that—GK can scramble and look foolish if he wants. The games I do have spare balls so won’t really delay anything on the other teams CK.)
In the real world, I think the only DR caution on a GK (or TI) is after the count down when the player is delaying the other team’s CK (or TI).

I think the OP question is can it be rather then would you? Fair enough if you don't but I would, especially after he moves it a second time, for a similar reason, as it would be obvious he is taking the pi55.
The OP is specific on "without five second countdown" so we are kind of limited here in terms of answers. From the strict wording of the law there would be no basis for a DR caution or for changing the restart to a corner kick. They both have the condition of countdown being up. But from a law intent, you can do either and should at least do one of them.
