So how many referees check every player's number against the teamsheet to see if they are wearing the right number before KO? In fact how many check some sort of photo ID to check the player on the field is who they say it is on the team sheet?
The fact of the matter is once they have given you a 'binding' document (or even verbal) you take their word for it and if it turns out to be incorrect then you report it.
Often just before KO I get told that player so and so is recorded as X on team sheet but is wearing Y because he forgot his jersey. I thanks them for it, record it on my game card and get on with the game. I only report it if a matter arises from it. The league has the least of interest in getting such trivial report (there will be hundred of them in grassroots). I can tell this for a fact and from experience.
Not my job to check that. Up to the manager to check IDs.

But if I can see that a player is on the field who is numbered on the teamsheet, then I immediately know there's a problem.
 
It's the obvious things. You cautioned the home 7 in the first half, you then caution the home 7 in the second half, but it is a different home 7. That isn't credible and those that don't know about the shirt change, such as opponents, spectators, etc, won't have a clue what is going on. Before you know it you'll be on social media as the referee that cautioned a player twice without sending off.

Give them a little more credit. The whole team changed shirts to different colours at half time. Surly they would have some idea. And how many players or spectators do you know who make a mental note of the shirt number of players who get cautioned? :)

It probably depends on the level. If there are paying spectators then it can't happen, there are team sheets showing players and numbers.

At lower levels it is still a problem. I try to judge persistent infringement using shirt numbers as I'm rubbish at recognising and remembering faces, but that approach is ruined if players come out for the second half with different shirt numbers.
 
I agree with @RustyRef on this, especially on the PI and cautions. I assisted a ref last season and they made a sub and the players swapped shirts. I stuck my flag up to call the ref over and informed him that the player coming on had swapped his shirt with the player coming off and was now number 7 (i thought as the ref, i would want to know this) he sort of chuckled at me and said errr right!!! After the game he asked me why i called him over for such a petty thing??
