Anyone know what the “problems” were that led to the change? I was on a long hiatus at the time and came back to the changed rule. I don’t recall ever having or seeing any problems, and players were allowed ahead of the ball for a very long time before that. Was the issue controversial OS scenarios with neither the R or AR in a position to make a call?

Depends how long ago your training course was.
The requirement for the "other" players to be behind the ball at a penalty kick was only introduced in 1995.

OK, then whoever was teaching your course obviously got that one wrong then.

Maybe they had learned the laws before 1995 and hadn't noticed the change? Seems a tad unlikely, but still.
