Should refs write the goal scorers in full time and there is a box for penaltys too little explanation please?

No, these are for the teams to fill in, not the referee. The referee may not have this information, but the teams will. It’s up to them if they want to fill these in - some do, others don’t. The teams are supposed to put down names of players who took part in the game but not all teams do this either.
