False reporting

Recently in a game I watched I witnessed a referee who had sent off a player for violent conduct but due to a couple of mistakes in the game, one of them leading to the violent incident in question, the referee said to that players manager I'll report it as two bookings instead of the violent conduct that the red card was originally issued for.

Has anyone else ever witnessed or had something similar and should the referee in question had been reported?

It happens a lot unfortunately. The referees often get themselves in trouble as the clubs query the reports and suspensions when they come through and it alerts the CFA to the fact that something is amiss.
