Correct restart for a foul when the ball is out of play

Of course, if the infringement happens when the ball is out of play, it wouldn't be a "foul"... just misconduct.

But I can't think of any either where the restart would _change_ to an IFK.

It depends... if the player left the field of play to commit the offence, then it's an IFK where the ball was.

If the player happens to be off the field of play, and then committed the offence, it's a dropped ball. Where the ball was.
 
At the taking of a penalty kick, after the whistle/signal is given, but before the ball is kicked

Kicker does an unsportsmanlike trick to fool the goalkeeper (stops during run up)

Or

Kickers colleague(s) encroach inside the 10 yards.........and a goal is not subsequently scored in either case

Restart with an idf to the defending side from the position of the offence, having first cautioned the kicker for the first one described.

As the ball hadn't been moved, the ball was still out of play when the offence was committed
