A Second Yellow Card - Wholegame

So I put the first yellow into cautions, and I have put "S7 - Receives a second yellow card in the same match" into dismissals. But dismissals has asked what the first yellow was for.

So should I remove the yellow from cautions, as both are now logged in dismissals?

Yes. Only report cautions for players who received one in the game in the cautions sheet.

A player receiving 2 in the game -excluding sin bins cautions - only needs to reported on the sending off form.

The relevant FA process it as a sending off, not a caution and a separate sending off.
