We've got three long away games in August - can't make Accrington on the opening day because I'm on holiday, but in the space of 8 days I'm doing the furthest north (Carlisle) and furthest south and furthest west (Plymouth). Probably about 26 hours on a coach in all - ouch!!

Has the defender's foot gone between the attacker's legs or to one side. The I cannot imagine a situation where the former occurs and it would not be a foul, but the latter should never be a foul merely because of the angle.
The ball was out of play when this incident happened, so as said above, free kick for the original foul is to be awarded. You can never reverse a free kick (it's not rugby!)
This is obviously more serious than most, but when a referee is assaulted I believe the police should get involved. Without criminal, external action, players will go on thinking it is acceptable as it is 'all part of the game'.
If you were to allow play to continue and something were to happen to him, think of the grief you'd get for allowing play to continue even though it's the manager's fault. I would strongly recommend a substitution, and if that does not happen I would abandon the match for a serious injury.
