Defender is shadowing the ball out from the attacker, they are in contact with each. The ball runs out of play and the 2 of them end up chest to chest and then the defender leans in with his head, only for a split second and it was only a lean in, no force.

Would you give a red for violent conduct?

Was there contact with the head? What was the speed of the lean motion? There will always be force. The leaning action itself needs force. It can be very little or negligible force but there will be some.

You'd have to be there. Temperature of the game would be a big factor on the decision.

Agree. Also reaction of opponent. If the opponent slugs him, they are both going off. If the opponent laughs and rolls his eyes, probably a caution. And in between, we have to referee.
 
this sounds strangely identical to my game last weekend. A clip from defender to attacker, both straight up handbags effing and jeffing, chest together head together but no 'headbutt' from either. Both as bad as each other personally, temperature of the game heating up so I elect to bring both in and captains and show both yellow cards for AA. I suppose it depends as others have said reaction from opponent even other players, if this happened bench side as mine did both shouting for something to be done.
