A team has to have a goalie on the pitch. If you've binned the gk then this will naturally be a stoppage in play. So they either put an outfield player in goal or you can allow a sub. It's entirely in your control to allow this.

This is interesting. The Law specifically says you award an IFK if the attacker is too close. I say interesting because there's no instruction for the referee to delay the kick until such distance is given nor the ability to retake for an offence.

As Mintyref suggests, go with what the referee wants. Often they won't want help with judging fouls - don't be offended; even though you are a referee in your own right the other CAR may not be so it keeps it simple.
