Just currently near the end of writing my red card forms from Wednesday, do I need to state that one of my send offs was a Yellow Straight Red? or do I put it through as a Straight Red?

If you're talking about a player getting cautioned then getting a straight red for another offence, you report them separately.

Report the caution, then do a report for the red card.

Yellows are yellows, reds are reds.

Double yellows are a a red detailing BOTH offences (in the 18th minute Mr X commited a Foul tackle against ...... for which i cautioned him under code C1 Foul Tackle. Later in the 63rd minute he ...... for which i cautioned him under code C5. as this was his econd caution in the game I showd him a red card under sanction S7 and dismissed him from the field of play and the surrounding vicinity).
