Wholegame system double caution

Not a new referee but a newbie question. I am entering a dismissal for a 2nd caution.... do I put in as a caution and then as a dismissal or just as a dissimisal?
I have previously done this as Caution and Dismissal, but I think this is duplicating the input? Any advice welcome.

Just as a dismissal, then when you select S7 it will ask you for the details of both cautions. That's my recollection anyway, haven't had an S7 for ages.
