Inspired by Vardy/Webb Park incident

The referee drops his whistle. A red player, standing in the center circle, picks it up and blows it just after a blue player has entered the red penalty area with the ball.
The whistle interferes with play and denies a goal scoring opportunity.

What's the sanction, if any, restart position and the restart?

I know most will know this, but same scenario except it’s a home ball boy that blows the whistle?
 
Someone in the technical area of opponents but you can't identify who it was.
 
Dropped ball where last touched to team that last touched it unless in PA yada yada
Interesting scenario that would cause chaos though if away team are on the attack with the ball in the penalty area, a home ball boy blows the whistle to put attacker off and we restart by dropping it to the home goalkeeper 😆
 
So if you can't identify the manager is classed as responsible and sent off? (Seeking clarity not questioning your answer)
Yes. Important to clarify that this is only for technical area offences and not for on field disciplinary.

Relevant text from law:

"Where an offence is committed by someone from the technical area (substitute, substituted player, sent-off player or team official) and the offender cannot be identified, the senior team coach present in the technical area will receive the sanction."
