Grayson submitted a new resource:

Matchpad - 5 subs, Numbered 1-17.

Thought I'd share the match record pad that I cobbled together for myself a season or two ago. Features numbered 1-11, with 12-17 for five subs (not #13).

Fits four to an A4 page if you print it properly - my notebook is A6 size (quarter of A4). If it works for you, feel free to use it!

And what is the box with "off" written in next to sub names?

Think the off bit is for the number of the player who comes off, although I may be wrong?

We have roll-on/roll-off subs down here, which renders any form pretty much useless.
I have carry two notebooks, one of the pre-printed variety to record goals etc, and a blank notebook for subs and discipline. So subs are "green 2/38 6 on 3 off" i.e green 6 came on for green 3 38th minute of the 2nd half, and discipline is "green 2 Harry Headcase 1/40 YC USB/FT" pretty much self explanatory.
The pre-printed forms just don't work for me.
