There are two downloads. Both designed for A4 size paper. One has 4 cards and the other 1 card.
The cards are sized to fit in a standard referee wallet. You can print them on any printer but I had them professionally printed on 300gsm cardboard so they are thin enough to be used in a wallet but thick enough to be used stand alone (the pen doesn't go through it when writing on it).
There is no space for names for sanctions as I don't takes names but there is ample space at the back of the card to take notes or write anything.
