What to send a caution in as

I gave a yellow today for DOGSO but with attempt to play the ball. What do I send it in as on whole game? Would it just be foul tackle?

Yes, just the actual offence that occurred. It's no longer DOGSO as the team gets an OGSO via the penalty you awarded. Therefore it doesnt need to be reported as DOGSO as thats neither here nor there in this instance.
